PRELIMINARY INFORMATION: NO PURCHASE NECESSARY. This Sweepstakes is only open to permanent legal residents of the forty eight (48) contiguous states of the United States and the District of Columbia who are physically located and residing therein, and who are eighteen (18) years of age or older as of July 17, 2008. Residents of the states of Alaska, Hawaii, Guam, Puerto Rico and all other U.S. territories and possessions are not eligible to enter this Sweepstakes. The Burn Notice Covert Ops Sweepstakes (“Sweepstakes”), which will take place in conjunction with the USA Network television show Burn Notice (“Show”), begins on July 17, 2008 at 10:00:01 p.m. ET and ends on September 25, 2008, at 9:59:59 p.m. ET (“Sweepstakes Period”). All times in these Official Rules refer to Eastern Time. Odds of winning depend on the number of eligible entries received.
TO ENTER: Beginning at 10:00:01 p.m. ET on July 17, 2008, coinciding with the scheduled broadcast of the Show, viewers will be invited to participate in the Burn Notice Online Game (“Game”) and Sweepstakes (Check local listings for exact dates and times when Show will be aired). Beginning on July 17, 2008 at 10:00:01 p.m., coinciding with each new episode of the Show, at least one (1) new mission will be introduced on the Web Site (see below chart for the Start Dates and Times of the New Game Missions). Burn Notice account holders who are registered participants in the Sweepstakes and who log in and successfully complete all nine (9) Game missions will be automatically entered into the Sweepstakes drawing (“Game Entry”). Game may also be played without registering for the Sweepstakes.
To enter the Sweepstakes by playing the Game, participants can access burnnoticeops.com (“Web Site”), log into his/her Burn Notice account by entering his/her password, clicking on the “Play Now” link, and then following the online mission instructions for each new mission. Prior to Game play, each prospective entrant will be prompted to complete the Sweepstakes registration information which consists of his/her first and last name, complete home address including city, state, ZIP code, daytime telephone number (including area code), email address, acknowledged acceptance of the Official Rules, gender, age and answers to the General Motors marketing questions (“Sweepstakes Registration”). Sweepstakes Registration must be completed in full the first time a Burn Notice account holder participates, and need not be completed again. Once the Registration information is submitted, the Burn Notice account holder will then click on the “Play Now” link and follow the online instructions to play a mission of the Game. Previous Game missions will be available on the Web Site for play/Game Entry during the entire Sweepstakes Period.
To enter the Sweepstakes by email without Game play/creating an account, send an email to webmaster@usanetwork.com. Information required in the email includes: your first and last name, complete home address (including street address, city, state and zip code), date of birth, telephone number (including area code), email address and the words “Burn Notice Online Game Entry” (“Email Entry”).
LIMIT OF ONE (1) ENTRY TOTAL DURING THE SWEEPSTAKES PERIOD: Regardless of the method of entry, you are limited to one (1) entry per person, email address or cell phone during the Sweepstakes Period. Multiple Game Entries or Email Entries (collectively, “Entries”) received from any person or email address beyond the stated limit will be void. All Entries become the property of Universal Television Networks, 30 Rockefeller Plaza, 21st Floor, New York, NY 10112; and NBC Universal, Inc., 30 Rockefeller Plaza, New York, NY 10112; (collectively, “Sponsors”), and General Motors Corporation, P.O. Box 33170, Detroit, Michigan 48232 (“Sweepstakes Entity”) and will not be acknowledged or returned. Entries must be received by 11:59:59 p.m. ET on September 25, 2008. Entries generated by script, macro, or other automated means will be void. Entries that are incomplete, garbled, corrupted or unintelligible for any reason, including, but not limited to, computer or network malfunction or congestion, are void and will not be accepted. Proof of Game play and/or delivery or receipt of Entry will not be deemed by Sponsors as proof of Entry. In case of a dispute over the identity of an entrant who submitted a potentially winning Entry, Sponsors reserve the right to deem that the Entry was made by the registered subscriber of the email account submitted at the time of Entry (per internet service-provider records required to be produced by the entrant) and such individual must comply with these Official Rules. Any personal information supplied by entrants to Sponsors will be subject to Sponsors’ and Sweepstakes Entity’s Privacy Policies and Terms Of Use posted at the Web Site. Except where prohibited by law or Sponsors’ and Sweepstakes Entity’s Privacy Policies, Entry constitutes permission for Released Parties (as defined below) and their respective designees to use entrant's name, city, state, likeness, image, and/or voice for purposes of advertising, promotion, and publicity in any and all media now or hereafter known throughout the world in perpetuity, without additional compensation, notification or permission.

ELIGIBILITY: Open only to permanent legal residents of the forty eight (48) contiguous states of the United States and the District of Columbia who are physically located and residing therein, and who are eighteen (18) years of age or older as of July 17, 2008. Entrants must have either a valid social security number and/or a valid U.S. personal tax identification number and possess a valid U.S. driver’s license issued by one of the fifty (50) United States or the District of Columbia. Residents of the states of Alaska and Hawaii and residents of Guam, Puerto Rico and all other U.S. territories and possessions are not eligible to enter this Sweepstakes. Officers, directors, and employees of Sweepstakes Entities, members of these persons’ immediate families (spouses and/or parents, children, and siblings, and their spouses), and/or persons living in the same households as such persons (whether related or not) are ineligible to enter the Sweepstakes. Sweepstakes Entities, as referenced herein, shall include Sponsors, Sweepstakes Entity, and their respective parent, subsidiary, and affiliated companies and administrative, advertising, and promotion agencies.
WINNER SELECTION AND NOTIFICATION: Fifty (50) first prize winners (Collectively, “First Prize Winners”) and one (1) grand prize winner (“Grand Prize Winner”) will be selected in a random drawing by employees or representatives of Sponsors from all eligible Entries on or about October 2, 2008. Sponsors will make two (2) attempts within a twenty-four (24) hour period to notify potential First Prize Winners and the Grand Prize Winner at the phone number submitted at the time of entry. Potential Grand Prize Winner (if a resident of a jurisdiction that deems winner to be a minor, his/her parent or legal guardian) will be required to execute and return (via shipping service pre-paid by Sponsors) an Affidavit of Eligibility, Liability Release, Publicity Release (where legal), a copy of his/her valid U.S. driver’s license issued by one of the fifty (50) United States or the District of Columbia, an IRS Form W-9 and any other documentation that Sponsors may require (collectively, “Sweepstakes Documents”) within five (5) days after receipt of these documents by the potential Grand prize Winner or the potential Grand Prize Winner may be disqualified and an alternate Grand Prize Winner may be selected from the remaining eligible Entries. By returning the completed Sweepstakes Documents, the potential Grand Prize Winner attests to the authenticity of his/her Entry. If a potential Grand Prize or First Prize Winner cannot be reached, if he/she is found to be ineligible, if he/she cannot or does not comply with these Official Rules, and/or if his/her prize or prize notification or prize correspondence is returned as undeliverable, that potential Grand Prize and/or First Prize Winner will be disqualified and an alternate Grand Prize and/or First Prize Winner may be selected from the remaining eligible Entries.
PRIZES/ODDS OF WINNING: First Prizes: There will be fifty (50) first prizes (“First Prizes”) awarded, each consisting of the following: One (1) pair of Oliver Peoples Sunglasses (exact model and style of sunglasses to be determined by Sponsors in their sole discretion). The First Prize offer is not affiliated with Oliver Peoples/Luxottica or any of its affiliates. The Approximate Retail Value (“ARV”) of each First Prize is three hundred and fifty dollars ($350). Total ARV of all First Prizes is seventeen thousand five hundred dollars ($17,500). Grand Prize: There will be one grand prize ("Grand Prize") awarded consisting of the following: One (1) 2009 Saab 9-3 2.0T Convertible. (“Vehicle”) and one (1) trip (“Trip”) for Grand Prize Winner and his/her guest (“Guest”) to Miami, Florida to visit to the Burn Notice set and a the opportunity to meet the cast crew of the Show. The ARV of the Vehicle is forty-three thousand, two hundred and eighty dollars ($43,280). The ARV of the Trip is two thousand seven hundred and fifty dollars ($2,750). The total ARV of the Grand Prize is forty six thousand thirty dollars ($46,030). Total ARV of all Prizes is sixty three thousand five hundred and thirty dollars ($63,530). Odds of winning a prize will depend upon the number of eligible entries received. Vehicle includes five speed automatic transmission, all season BSW tires, leather appointed sport bucket seats, black convertible top, AM/FM stereo with CD player, and 16” x 6.5” 14-spoke alloy wheels. Any differences between the stated ARV of the Vehicle and the actual Manufacturer’s Suggested Retail Price (“MSRP”) of the Vehicle will not be awarded. Any depiction of the Vehicle is for illustrative purposes only. No substitution, transfer, or cash equivalent will be awarded in lieu of Grand Prize, except that Sponsors, at their sole discretion, reserve the right to substitute Grand Prize (or portion thereof) of comparable or greater value for any reason. Vehicle will only be delivered to an address within the forty-eight (48) contiguous states of the United States. Winner (if a resident of a jurisdiction that deems winner to be a minor, his/her parent or legal guardian) must take delivery of the Vehicle at a location designated by Sponsors in their sole discretion (“Delivery Site”). Sponsors or their representatives reserve the right, but are not obligated, to designate the Delivery Site as the Grand Prize Winner’s residence or a dealership close thereto. Sponsors will use all reasonable efforts, subject to availability, to deliver the Vehicle within ninety (90) days of receipt of a completed Sweepstakes Documents (defined above). Sponsors are not responsible for any delays in the delivery of the Vehicle. In order to accept the Vehicle, winner must possess and present adequate personal identification, evidence of legally required insurance, and a valid U.S. driver’s license issued by one (1) of the fifty (50) United States or the District of Columbia. Winner bears the risk of damage to Vehicle after delivery to specified Delivery Site and Vehicle will be forfeited if not picked up within thirty (30) days after delivery thereto. Vehicle shown may not be actual Vehicle delivered. All reasonable efforts shall be used to provide the same model and year as shown or at least a Vehicle having substantially the same dealer cost. Winner is solely responsible for all travel and transportation costs incurred in connection with collecting Vehicle, as well as any sales tax, luxury tax, title and license fees, registration fees, insurance, additional options, and any other costs incurred in claiming, registering, or using the Vehicle, including, without limitation, any costs incurred in bringing the Vehicle from the Delivery Site to the Grand Prize Winner’s home and any other costs/expenses not specifically mentioned as being included. Vehicle will be delivered solely with the express written limited warranties set forth in the applicable Warranty Booklet. Grand Prize Winner is not eligible for any cash rebate programs. Interior/exterior colors of the Vehicle will be determined by Sponsors in their sole discretion. Winner agrees that General Motors has neither made nor is responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to the Vehicle, including, but not limited to, the quality, mechanical condition, or fitness, except that Vehicle does include General Motors’ standard new vehicle limited warranty as set forth in the Vehicle’s Warranty Booklet (see Saab dealers for full details). All federal, state, or other taxes, including, without limitation, income taxes, are the sole responsibility of the Grand Prize Winner. Trip includes round-trip, coach-class air travel from a major commercial airport near Winner’s residence (as determined by Sponsors in their sole discretion) to a major commercial airport near Miami, FL (as determined by Sponsors in their sole discretion) (“Miami Airport”); standard hotel accommodations (single room/double occupancy) for three (3) nights at a hotel (as determined by Sponsors in their sole discretion) (“Hotel”); the opportunity for Grand Prize Winner and his/her Guest to visit to the Burn Notice set and to meet the cast crew of the Show (exact date and time of visit to be determined by Sponsors in their sole discretion) (“Visit”); ground transportation for two (2) between Miami Airport and Hotel; and ground transportation for two (2) between Hotel and location of Visit. Winner and Guest must be able to depart on dates and times determined by Sponsors in their sole discretion or Trip portion of the Grand Prize (including Visit) will be forfeited and no additional compensation will be awarded. Grand Prize Winner and Guest must travel together on the same itinerary. Grand Prize Winner will receive an IRS Form 1099 from Sponsors for the total ARV of the Grand Prize for the calendar year in which Grand Prize was won. Sponsors reserve the right to substitute a similar Grand Prize or First Prize (or prize element) of equal or greater value for any reason at their sole discretion. All taxes and other expenses, costs, or fees associated with the acceptance and/or use of the Grand Prize are the sole responsibility of the Grand Prize Winner. Prizes cannot be transferred by Winners or redeemed for cash and are valid only for the items detailed above, with no substitution of a Prize by a First Prize or Grand Prize Winner. If a prize is unclaimed, it will be forfeited and an alternate winner may be selected from the remaining eligible Entries. Once travel has commenced, if a Grand Prize element (including, without limitation, attendance at the Visit due to Show cancellation or postponement, or any other reason) is not available for any reason, as determined by Sponsors in their sole discretion, or, if Grand Prize Winner chooses to take the Trip and/or attend the Visit without a Guest, the remaining elements of the Grand Prize shall constitute full satisfaction of Sponsors’ obligation to Grand Prize Winner and no additional compensation will be awarded. Travel and Hotel arrangements will be determined by Sponsors in their sole discretion. Once determined, no change, extension, or substitution of Trip dates are permitted, except by Sponsors in their sole discretion. Guest must be eighteen (18) years of age or older and the age of majority in his/her state of residence as of July 17, 2008 or Winner must be Guest’s parent or legal guardian and must complete and return all releases on behalf of minor. Grand Prize consists only of the elements set forth in these Official Rules. No other travel elements (including, without limitation, ground transportation at point of origin, insurance, meals, gratuities, souvenirs, etc.) are included in Grand Prize. All taxes and fees assessed at the time of ticketing are the responsibility of Grand Prize Winner. Tickets are non-refundable and are not valid for business-related or group travel. Tickets are valid only for travel on airline chosen by Sponsors in their sole discretion. Tickets will be issued only to Grand Prize Winner and his/her Guest. Tickets are void if sold for cash or other consideration. Grand Prize Winner is responsible for any tax liability, including applicable departure taxes or fees, baggage fees, inspection charges, and security charges. Once issued, tickets are non-transferable and may not be reissued once travel has commenced. Open tickets will not be issued and stopovers are not permitted. Travel arrangements must be made through Sponsors. Grand Prize Winner and his/her Guest must have valid travel documents (e.g., valid, government-issued photo identification and/or passport) prior to departure. Sponsors shall have no responsibility or liability for cancellations, delays, or any other change by any company or person providing any element of the Grand Prize due to reasons beyond Sponsors’ control, and are not responsible or liable for any expenses incurred as a consequence thereof. Travel may not be combined with any other offer. All travel will be at the risk of the Grand Prize Winner and his/her Guest. Guest, if any, will be required to execute and return a release of liability and, except where prohibited, publicity release, which must be returned with the Sweepstakes Documents, or Grand Prize will be forfeited and an alternate Grand Prize Winner may be selected from the remaining eligible Entries. Any depictions of prizes are for illustrative purposes only. Prize notifications and Sweepstakes Documents will be mailed/shipped to a street address within the forty eight (48) contiguous states of the United States only. Sweepstakes administrator is not the supplier or guarantor of Prizes.
CONDITIONS: By entering the Sweepstakes, each entrant agrees for entrant and for entrant’s heirs, executors, and administrators: (a) to release and hold harmless Sweepstakes Entities and their respective officers, directors, employees, shareholders, representatives, and agents (collectively, “Released Parties”) from any liability, illness, injury, death, loss, litigation, or damage that may occur, directly or indirectly, whether caused by negligence or not, from such entrant’s participation in the Sweepstakes and/or his/her acceptance, possession, use, misuse, or inability to use Prize; (b) to indemnify Released Parties from any and all liability resulting or arising from the Sweepstakes and to hereby acknowledge that Released Parties have neither made nor are in any manner responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to Prize, including express warranties provided exclusively by the Sweepstakes Entity that are sent along with Prize; (c) if selected as the Winner, to the posting of such entrant’s name on and the use by Released Parties of such name, voice, image, and/or likeness for publicity, promotional, and advertising purposes without additional compensation (except where prohibited), and, upon request, to the giving of consent, in writing, to such use; and (d) to be bound by these Official Rules and to waive any right to claim any ambiguity or error therein or in the Sweepstakes itself, and to be bound by all actions of the Sponsors, whose decisions are binding and final. Failure to comply with these conditions may result in disqualification from the Sweepstakes. In the event of a dispute as to the identity of an entrant, the authorized account holder of the email address used to enter, if applicable, will be deemed to be the entrant. “Authorized account holder” of an email address is defined as the natural person who is assigned to an email address by an Internet Service Provider, on-line service provider, or other organization (e.g., business, educational institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted email address.
ADDITIONAL TERMS: Sponsor reserves the right to terminate the Sweepstakes or permanently disqualify from any promotion any person they believe has intentionally violated these Official Rules. Any attempt to deliberately damage the Sweepstakes or operation thereof is unlawful and subject to legal action by Sponsors, who may seek damages to the fullest extent permitted by law. Sponsors reserve the right to cancel or modify the Sweepstakes should any computer virus, malfunction, or other cause beyond their control corrupt the administration, security, or proper operation thereof. In the event of cancellation, Prize will be awarded from among all eligible, non-suspect Entries received prior to the event requiring cancellation. Notice of cancellation will be posted at The failure of Sponsors to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, terrorism, act of public enemies, actions of governmental authorities outside of the control of Sponsors (excepting compliance with applicable codes and regulations), or other “force majeure” event will not be considered a breach of these Official Rules. Released Parties assume no responsibility for any injury or damage to entrants’ or to any other person’s computer and/or telephone relating to or resulting from entering or downloading materials or software in connection with the Sweepstakes. Released Parties are not responsible for telecommunications, network, electronic, technical, mobile telephone service, or computer failures of any kind; for inaccurate transcription of Entry Information; for errors in any promotional or marketing materials or in these Official Rules; for any human or electronic error; or for Entries that are stolen, misdirected, garbled, delayed, lost, late, damaged, or returned.
DISPUTES: The Sweepstakes is governed by, and will be construed in accordance with, the internal, substantive laws of the State of New York, and the forum and venue for any dispute shall be in New York, New York. If THE controversy or claim is not otherwise resolved through direct discussions or mediation, it shall THEN be resolved by FINAL AND binding arbitration administered by JUDICIAL ARBITRATION AND MEDIATION SERVICES, INC., in accordance with its Streamlined Arbitration Rules and Procedures or subsequent versions thereof (“JAMS Rules”). The JAMS Rules for selection of an arbitrator shall be followed, except that the arbitrator shall be experienced and licensed to practice law in new york. All proceedings brought pursuant to this paragraph will be conducted in the County of new york. THE REMEDY FOR ANY CLAIM SHALL BE LIMITED TO ACTUAL DAMAGES, AND IN NO EVENT SHALL ANY PARTY BE ENTITLED TO RECOVER PUNITIVE, EXEMPLARY, CONSEQUENTIAL, OR INCIDENTAL DAMAGES, INCLUDING ATTORNEY’S FEES OR OTHER SUCH RELATED COSTS OF BRINGING A CLAIM, OR TO RESCIND THIS AGREEMENT OR SEEK INJUNCTIVE OR ANY OTHER EQUITABLE RELIEF.
